A few changes.

- You no longer have to use INDEX (ex. GHELP INDEX) to get a listing
of the help files. Just GHELP in this instance will list them all.

- You can now use spaces in the help file names. You can also use
underscores, but they will be converted to spaces.

- The index file will be converted to use two columns rather than
just one. This will happen the next time you edit one of your orghelp
files.
